Carotid Ultrasound (Intima-Media Thickness / Plaque)

Last reviewed 25 August 2026 · Evidence-graded review — see Methodology

Ultrasound assessment of carotid artery wall thickness and any plaque, frequently marketed as a “vascular age” or stroke-risk test. Carotid plaque does reflect systemic atherosclerosis, and high-grade stenosis can directly cause stroke — which is what makes the sales pitch plausible and the guideline position worth reading carefully.

Why it grades Weak

The US Preventive Services Task Force gives screening for asymptomatic carotid artery stenosis a “D” recommendation — that is, actively against it — with moderate certainty that the harms outweigh the benefits. Those harms are unnecessary downstream imaging and procedures arising from false positives, plus procedural complications from unindicated intervention. No externally validated method reliably identifies which asymptomatic adults are genuinely at increased stroke risk from an incidental carotid finding.

USPSTF Recommendation Statement, JAMA 2021. PubMed: PMID 34058106

Commonly sold in executive-health packages as a "vascular age" scan. The relevant professional guideline explicitly recommends against this in asymptomatic adults — one of the clearer cases of a marketed test running against consensus evidence.

Not covered in the current review

Singapore’s national screening committee has not reviewed this test in the current volume of its report, which covers nine conditions. That is not a verdict either way.

Not documented

Not covered in the current volume of the committee’s report, so no harm criterion was found either way.

An empty finding, not a clean bill of health: this means nothing documented was found for this test. It is not a claim that the test is safe.

Screening harm is not only a laboratory question: the committee’s own criteria advise against a test when “the screening test, or follow-up tests arising from a positive screen, are associated with significant medical risks”. Sources on overdiagnosis → · How this is graded.

FAQ

Is the Carotid Ultrasound (Intima-Media Thickness / Plaque) worth doing in Singapore?

Carotid Ultrasound (Intima-Media Thickness / Plaque) holds a weak evidence grade under our screening rubric. It typically costs S$150–300 in Singapore. It is most appropriate for patients with focal neurological symptoms (TIA/stroke workup) or a carotid bruit on exam — not general screening. It is not recommended for asymptomatic screening.
